Our Specialist Care service has already successfully supported individuals who have a range of needs and we support people who have:
- Mental health conditions, including anxiety disorder, bi-polar affective disorder, and personality disorder.
- Learning disabilities and health needs, including Challenging Behaviours, Complex Autism, people with communication barriers.
- Acquired Brain Injury, and many more
Last year, SCA Care launched a brand new bespoke Specialist Care service supporting people with learning disabilities, mental health conditions, physical disabilities and other Specialist Care needs. We always place the individual at the heart of everything we deliver through person-centred planning, including the way in which support is implemented, recruited for and delivered.
We pride ourselves on taking a proactive approach which focuses on supporting individuals to manage their own mental wellbeing. We work in partnership with the local community and support clients to build positive support networks to help them reach their goals.
Our support is flexible to suit the individual’s needs. We cover a range of services such as:
- Assisting individuals with day-to-day tasks in their home setting, including maintaining a household, budgeting, healthy living and health needs.
- Supporting clients to become a valued and integrated member of their community.
- Outcome-based support through person-centred planning to ensure that people reach their full potential. We work with individuals in partnership with other professionals to develop: Positive behaviour plans, Health and wellbeing plans, WRAPs (wellbeing recovery action plans), Health management plans.
- Supporting individuals to transition from inpatients of long term medical and care facilities and from their family setting into their own homes.
- Ensuring people have a smooth and successful transitional process that is carried out at a pace suitable to both the individual and commissioners.
- Using the innovative Pass Care Management System which provides a live view of care records including enquiries, medication prompts, tasks, reporting and assessments. This platform also gives reassurance to families and professionals who can securely log into a person’s care record remotely through the OpenPASS app.
- We have experience working in home settings with complex family and environmental dynamics, always ensuring that individual’s circle of support is maintained and developed.

Stephanie Kitcher, Specialist Care Manager
Stephanie has worked within health and social care for over 10 years, helping to support individuals with learning disabilities, complex health and mental health needs. She has successfully built positive relationships with partnering organisations in the care sector and always ensures that a positive outcome is achieved for the client.
Case studies
Ms B
Ms B is non-verbal and lives at home with her parents. Ms B has a vibrant personality and enjoys being out and about in the same social circles.
During Ms B’s transition to our service we spent time working with her and her family to understand what style of support would best suit her and we identified the support staff within our team that would be the best match.
Despite Ms B’s complex health conditions, we have supported her on week-long respite breaks both within her own home while her family are away, out at social nights like the Edge in Southampton, where she really enjoys dancing.
Our support workers have successfully integrated into the family setting, while simultaneously ensuring the family’s independence. An example of this was Ms B’s family’s desire to support Ms B to try out an indoor skydive experience. The family planned this in depth and liaised with the centre to ensure it could accommodate her needs. It was clear at the event that the support staff were there to support her, while also blending into the background without intruding on the experience.
Mrs A
Mrs A was referred to our service after a short period of time in a psychiatric hospital where she was diagnosed with Personality disorder. Our team worked closely with healthcare professionals to understand her medical and social requirements. We identified that Mrs A wanted support to help manage her flat, as well as medication management, support to cook, building her confidence in the community, and finding meaningful activities in the local area. She was also extremely anxious about getting back into her local community following her relapse and we were determined to help her build her confidence.
After a period of introduction to Mrs A, the staff team were able to support Mrs A to deep clean her home, purchase new items of furniture for the flat, and build a cleaning routine around her daily life. As a result she became more independent. We also built on her existing skills to enable Mrs A to become independent with taking her medication, and she is now able to manage this task independently.
Because of the work that we have done with Mrs A has regarding building her confidence, the number of support hours Mrs A required have been reduced and our focus has turned to her other goals and priorities. We are proud to say that Mrs A is now working towards finding a volunteering role and her independence has resulted in less calls to other professionals, including emergency services.
